Other Information
All patients, please bring insurance card with you.

If you were referred by your primary physician,
have him/her fax us a referral letter and any other pertinent information.

Please let us know if your insurance company requires treatment at a specific hospital or if it uses a specific laboratory for any testing that might be done in our office.
