It is important that you are familiar with your own insurance plan so that you will know what to expect when your medical expenses arise. You should be aware that some insurance policies do not pay all services in full, in fact, most cases there is an amount left unpaid. The amount left unpaid is determined by the terms of your contract. If you have questions regarding whether or not your bill will be paid by your insurance, please contact them directly.

Just like any business our practice requires you to understand your financial obligations. Please take a moment to download, read, and sign our Patient Information Form which states our financial policy. We require all patients seen in the office to sign the financial policy prior to being seen by our physicians.
